Write your email address, name (first and last, no initials), complete street address (P.O. boxes will not be accepted), city, state and ZIP code, date of birth and telephone number on a 3½” X 5” post card. Cards must be postmarked between May 23, 2012 and July 2, 2012. Five (5) game codes will be emailed to the email address you provide.
